💼 Job Title: Medical Coder – HCC (Risk Adjustment) 
📌 Job Description
We are looking for a detail-oriented and experienced Medical Coder (HCC) with over 6 Months of hands-on experience in Risk Adjustment coding. The ideal candidate will be responsible for reviewing patient medical records, identifying chronic conditions, and assigning accurate ICD-10-CM codes to support proper risk score capture and healthcare compliance.
The role requires strong analytical skills, attention to detail, and the ability to ensure coding accuracy while adhering to CMS guidelines. Candidates with exposure to auditing, quality checks, or SME support will be an added advantage.
🎯 Key Responsibilities
    •    Review and analyze patient medical records for HCC coding
    •    Assign accurate ICD-10-CM codes for chronic and acute conditions
    •    Ensure proper documentation supports code assignment
    •    Follow CMS risk adjustment guidelines and compliance standards
    •    Identify missed HCC opportunities and documentation gaps
🧠 Skills Required
    •    Strong knowledge of HCC Coding & Risk Adjustment
    •    Good understanding of ICD-10-CM guidelines
    •    Knowledge of medical terminology & anatomy
    •    High attention to detail and accuracy
    •    Analytical and problem-solving skills
📚 Experience & Qualification
    •    Minimum 6 Months experience in HCC coding
    •    Life Science / Medical / Paramedical background preferred
    •    Certification (CPC / CRC) is an added advantage
